Candace DeLapp find that a few patients have these small chips that are occuring.  The first element to consider is your teeth are not tools!  More than one patient uses their teeth to open a variety of objects and these small chips can occur.

Chipping your bite because of your occlusion

How your teeth fit together is another element that can cause chipping of teeth especially the front teeth.  Moving your teeth  back and forth may show varying degrees of malocclusion that may be responsible for these chips. In “some” cases simple occlusal adjustments may reduce the risk of these chip re-occurring.  For “other” cases more complex adjustments or treatment may be necessary.

Repairing these small chips on your teeth

Repair of these small chips can be difficult in the long term.  In most cases a composite material is bonded to the tooth. While this material is adhesive shearing forces placed on these white dental fillings can cause the material to de-bond.  Repair again is made usually involving more of the tooth each time. Dental veneers or dental crowns may end up being a longer term solution to these chipping problem

What to do with a loose temporary dental crown?

A loose dental temporary crown is a common finding in most dental offices. Our Denver dentists cement these temporaries with temporary cement a.k.a. weak cement.  The overall idea is for the cement to last the two weeks while the molds of your tooth are being fabricated into your dental crown.  It is a simple matter to cement the temporary with a more durable cement but then getting the temp off to cement the porcelain crown would be challenging to say the least.

The temporary dental crownhas many functions.  First and foremost is it protect the surface of the tooth that has been exposed in the crown preparation.  For many but not all patients this exposed dentin is sensitive to hot and cold. Patients with this type of sensitivity there is not a problem with them returning and getting it re cemented.  The issue is when the temp comes off and it is NOT sensitive and the patient is thinking that they do not want to bother us and will just wait until their appointment to have the porcelain crown cemented.  While that is thoughtful … it frequently complicated the dental crown seating process.  First…. the gum can grow up over the margin on the dental crownpreventing the seating of the crown.  The second issue as to do with tooth movement of either the teeth tilting or super-erupting in to the crown preparation space.  If left long enough the NEW crown will not fit and have to be re-made.

Toothpaste as a temporary cement

On occasion despite your best intention you are not able to have the temporary re cemented.  Using a small amount of toothpaste on the inside of the crown “may” help retain the tooth until you can schedule an appointment.  Care must be given not to swallow or inhale the temporary at night while sleeping and it is recommend that it be removed at bedtime

What to do when a dental crown falls off

The answer to this question depends on what ultimately the problem is. Denver dentists Dr. Sarah Parsons and Dr.’s James and H. Candace DeLapp have one strong recommendation… get in ASAP. If it is something as simple as the crown cement getting loose then cementing again with dental cement is the first priority. Sometime dental cavitiescan sneak underneath the crown and weaken the tooth and the crown falls off. This involves re-making the crown and in severe instances may result in loosing the tooth.

Your best bet is to call and see a our Denver area dental office immediately. If you leave the dental crown off the tooth because it does not hurt your teeth ”will” move!  The question is how much and how fast.  If the tooth moves  we “may not” be able to place the same crown on the tooth. Spaces can become narrower or the upper toothcan erupt into the empty space and there in not enough room for the crown to fit in to. In “many” instances we need to fabricate the dental crown.

There are many different temporary dental cements that you can use. Some you can purchase at the Drug store. You can even use tooth paste as a temporary adhesive.  Our recommendation is to take the crown out at night so you do not swallow (or inhale) the dental crown.

Yesterday our Denver dentists Dr. James DeLapp up-loaded his new dental practice video on YouTube and am including it in this blog posting.

Our Denver area dental practice is in the process of developing a number of video describing dental procedures and answering dental questions with video that we upload to YouTube.  Unlike the video you see above… these video will be developed in house with nothing more complex that a “Flip” hand held video camera.  The first video we are planning on producing is on teeth whitening and in particular difficult teeth to whiten.  If you have been following our Blog postings you would know that the gray hue’s are difficult to whiten.  In addition an type of translucency can be challenging at best.

Recently updated our auxiliary web site for Parker Dentistry.  Information included was adding Dr. Sarah Parsons to the doctors who deliver treatment and adding the fact that we can offer “minimal” sedation dentistry to patients in the Parker and Castle Pines area of Colorado. Types of Sedation Techniques used by our Denver Dentist

There are a variety of techniques used in dental sedation… some are quite simple and some are much more complex.  As the complexity increases so does the extent of training as each inherent risks that each patient should be aware of and accept. While exceedingly safe as a general rule, there are rare instances in which patient can become more sedated than planned.  Your Denver sedation dentist needs to be properly trained on how to handle these situations. You should consider the technique that allow you to  accomplish the dental treatment. The general types of sedation include but are not limited to:

  • Psycho-sedation (discussion with the patient in a calm reassuring tone etc.)
  • Nitrous oxide oxygen dental sedation (laughing gas)
  • Oral Dental Sedation (anxiolysis)
  • Oral Pre-medication possibly combined with Nitrous oxide as an example (minimal sedation)
  • Intravenous Dental Sedation (moderate sedation)
  • Deep sedation
  • General Anesthesia in a hospital setting

Denver Oral Dental Sedation ( Minimal Sedation)

One of the most common dental sedation medicine categories is the family of benzodiazopines. Diazepam (Valium) is the most familiar drug in this category.  This medication has been used for years for anxiety and sedation.  Triazolam another benzodiazepine was approve to be used as a hypnotic in 1982.  It has been used in dentistry for some years and has a certain amount of anteograde amnesia.  What this means is “many” but not all patients, may not remember all the details of the dental procedure.  This has been termed in the general public use as” travelers amnesia”.  Not remembering where your hotel is can be a… not so good thing… but not remembering the details of your dental appointment may be a good thing. Triazolam is contraindicated in pregnant females.  Do not combine Triazolam with alcohol or other drugs unless approved by your doctor.  Operating vehicles or dangerous machinery is also not recommended.

Denver Cosmetic Dentist Dr. Sarah Parsonsrecently had a patient presnt with a smile they were not happy with.  It was a combination of the size … shape and color of the teeth. Dental photographs were  taken and evaluated.  As you can see below there are areas in which cosmetic dentistry could be of value. Dr. Parsons sent models of her case to Las Vegas Esthetics a premiere cosmetic dental laboratory for a diagnostic wax-up.  A diagnostic was up takes models of patients teeth and waxed up the proposed results.  The patient approved the wax up and the case was begun Below is the before photograph.

Tooth Sensitivity in Denver

Treatment of tooth sensitivity from exposed root surface.

A recent article in the Journal of the American Dental Association (JADA) discusses dentin sensitivity of teeth and what treatment was most effective. Our Denver dentists Dr. James DeLapp, Dr. H. Candace DeLapp and Dr. Sarah Parsons have used a variety of techniques to reduces tooth discomfort for their patients. The causes of root sensitivity would be another Blog article or more because there are so many reasons why the root exposure. The key is does the tooth sensitivity affect what you eat or drink.

Treatment of dentin sensitivity

In the past some of the treatment of tooth sensitivity included controlling your diet.  In this study the participants felt that controlling your diet was the least effective method of “treating” tooth sensitivity. Treatment that was reported to be most effective included:

  • Gluma desensitizer- a topical agent applied to the root surface
  • Dura Flor a fluoride gel placed on the dentin of the tooth
  • Prevident Gel a prescription based desensitizing solution
  • MI Paste combination of many agents to decrease tooth sensitivity
  • Tooth Bonding agents

First “try” over the counter teeth desensitizing agents

For “minor” root sensitivity you might want to try some over the counter agents. Sensodyne is most noted product to use.  Our strong recommendation is to see a dentist “first” to make sure you tooth sensitivity is not something more serious.  Sensitivity from cavities will not respond and are likely to become worse.
